Overview

Thereis an unmet clinical need to identify oropharyngeal cancer patients who are likely torecur following curative treatment. Here we propose to understand to what extent HPV heterogeneity influences cancer-specific outcomes, the influence this has on the tumour microenvironment and how this mightlead to recurrent disease.

Cancers of the head and neck are the 8th most commonly diagnosed cancer in the UK. Despite improvements to clinical practice and the recognition of HPV-related disease as a separate entity, predicted five-year survival is often less than 50% (PMID: 33243986). HPV-positive disease has been shown to respond favourably to standard of care, however, research from our group has previously demonstrated how use of a surrogate biomarker can influence patient staging and outcome in oropharyngeal cancers when definitive HPV testing is not carried out (PMID:30890775and 31666283). Understanding a patient’s HPV status may become clinically relevant for treatment stratification in the future,as it has been speculated that patients with HPV-related disease may benefit from treatment de-escalation to minimise long-term side effects, however, to date there has been limited successin these patientsdue to disease recurrence. Therefore,there is an unmet clinical need to characterise HPV heterogeneity within oropharyngeal cancer patients who are likely torecur following curative treatment. This study aims to understand to what extent HPV heterogeneityinfluences cancer-specific outcomes, the influence this has on the tumour microenvironment and how this might lead to recurrent disease.
