Progress to date

A new proforma for the audit has been designed and initial feedback was supplied from clinicians, statistician, and TVOs, and appropriate adjustments made. Completed on 9 October 2012.

1. As an audit pilot study, the TVOs have pulled the charts of 12 patients, and a subsequent meeting has reviewed the proforma performance making changes were desirable and referring unclear issues to NICR Director or clinicians. Completed on 5/11/2012.
2. The current phase of the study from November 2013 to May 2013 is the data collection.

This audit has now been completed and the report published in ####.
